This form is for TLAC members requesting a change to an existing membership.
Membership Changes & Cancellations
- Use this form for upgrades, downgrades, cancellations, or other membership updates.
- All membership changes must be submitted through this form so our team can properly review, track, and process the request.
- The front desk is not able to make membership changes directly.
Restarting a Previous Membership
- If you previously had a TLAC membership or purchased a day pass and already have an account with us, you can use this form to restart your membership remotely.
- If you are at the club, the front desk can assist with restarting a membership in person.
What Happens After You Submit
- After submitting the form, you should receive a confirmation email at the email address you enter below.
- If you do not receive a confirmation email, the form was likely not completed successfully or the wrong email was entered.
- If your form was submitted successfully, please allow up to 72 hours for our team to review your request and follow up.
- Submitting this form does not guarantee immediate changes to your account.
- Your membership will remain active under its current terms until your request has been reviewed and confirmed by TLAC.
