Use this form to upgrade, downgrade, add a family member, manage add-ons, or cancel your membership. You will get a confirmation email as soon as you submit - that email is your confirmation, so there's nothing to wait on unless we have a question.

Before you submit

All membership changes must go through this form. The front desk cannot make changes directly. This ensures every request is properly reviewed and tracked.
Notice windows matter
Monthly: 7 days notice required. Yearly: 30 days notice before your contract end date. Submitting outside the window may delay your change by one billing cycle.

You will receive a confirmation email right after submitting; that email is your confirmation, and we will only reach out if we have questions.

Days - Monthly contracts Required before the end of the month. For example: submit by June 24th to cancel effective July 1st.

Days - Yearly contracts Required before your contract end date. For example: if your contract ends July 1st, submit by June 1st.

Good to know
Changing membership type If you switch to a different membership type on a yearly contract, your one-year term restarts from the change date.
Restarting after cancellation A $50 enrollment fee applies any time a membership is restarted - even if you've been a member before.
