All membership changes must go through this form. The front desk cannot make changes directly. This ensures every request is properly reviewed and tracked.
Notice windows matter
Monthly: 7 days notice required. Yearly: 30 days notice before your contract end date. Submitting outside the window may delay your change by one billing cycle.

Days — Monthly contracts Required before the end of the month. For example: submit by June 24th to cancel effective July 1st.

Good to know
Changing membership type If you switch to a different membership type on a yearly contract, your one-year term restarts from the change date.
Restarting after cancellation A $50 enrollment fee applies any time a membership is restarted — even if you've been a member before.
